I pause MyDefrag and it says:
"Paused, press space to continue..."
Yes even after waiting a minute the app is still maxing out the CPU in Windows Task Manager.
I have a big 3TB drive almost full. Can it just not handle large drives?

MyDefrag won't pause the instant you pause it: it will finish moving the file (or a part of it) it is currently working on and then pause.
But moving files does not take up much CPU time. Is this reproduceable at any point in MyDefrag? Are you sure it's the MyDefrag process that's causing the the high load?
