(1) Sometimes I want specific files to be faster than others (like some of my huge PST files, but not all), and others slow (like a specific backup file). It would be nice to be able to tell MyDefrag to place a specific file at the beginning / end of disk.

(2) I can defrag my page file with Mark Russinovich's PageDefrag. However, it places the new pagefile where it wants -- in my case it placed it at the very end of the disk. No surprise -- that was where MyDefrag left enough free space. I see no way to move it at the beginning of the disk. As a solution, I would suggest to tell MyDefrag to leave enough free and totally unfragmented space as close to the beginning of the disk as possible, and then I would run PageDefrag in hope that pagefile will be moved there.

As a workaround, I could create a dummy file of the same size and ask MyDefrag to put it at the beginning of the disk (as in my first suggestion above). Then I would delete it and use PageDefrag.

Same thing with all other special files, like registry hives.

MyDefrag can already do all that, for more information see MyDefrag scripting.

Note that the resulting gap may contain unmovable files like special NTFS system files which MyDefrag can not move. Dependant on how you move the temporary file to the beginning, MyDefrag will wrap this file around unmovable files.
